Các đội bóng Giải bóng đá Ngoại hạng Anh 2003–04

Hai mươi đội thi đấu trong giải đấu – mười bảy đội hàng đầu của mùa giải trước và ba đội thăng hạng từ First Division. Các đội thăng hạng là Portsmouth, Leicester City và Wolverhampton Wanderers, trở lại giải đấu hàng đầu lần lượt sau mười lăm năm, một và mười chín năm vắng bóng. Đây cũng là mùa giải đầu tiên của Portsmouth và Wolverhampton Wanderers tại Premier League. Họ thay thế West Ham United, West Bromwich Albion và Sunderland sau thời gian thi đấu ở giải đấu hàng đầu lần lượt trong 10, 1 và 4 năm.
